• Long Haul, Owner Operators, For Hire
• New Ventures (min 3 years prior experience)
• Drivers must have US license and minimum 1-year Class A CDL
• No mileage restrictions (+300 miles)
• No minimum or maximum size
• No growth limitations
• Vehicle age 15 years and newer
• 26k GVW +

Ineligible/Prohibited Risks:

• Dump Trucks
• Public Transportation
• Wreckers, Towing Operations including Repossession
• Team Driving (unless specifically declared and added)
• Hazardous Material Haulers
• Hay, Logs, Pulpwood
• Sand/Gravel, Ready Mix, Frac Sand
• Major Violations are unacceptable

Coverages:

• Auto Liability: Up to $1M CSL
• UM/UIM: Statutory Limits
• No- Fault (PIP or Med Pay): Statutory Limits
• Hired Auto
• Non-Owned Auto
• UIIA Endorsement

Submission Requirements:

Completed Trucking Application
ALL driver MVRs currently valued within last 35 days (no exceptions)
3 years currently valued Loss Runs within last 35 days
Copy of the last 4 quarterly IFTA statements, if not all 4 then at least the most recent statement unless new venture
Valid DOT # required for the risk you are quoting
